The Summa Health Weight Management Institute makes it easy for you to refer a patient. Choose from the following options:

1. Fax: Fax a referral form (Download PDF Word) to:

2. Print your referral from your electronic medical record (EMR) system and fax to:

3. Use Join the Network (JTN)/P2P if your practice is registered for:
   
   • Surgical Weight Loss
   • Nonsurgical and OP Nutrition Services
4. Call the Summa Health Weight Management Institute to get the referral process started.

5. If you are a Summa Health Medical Group Physician using your EMR, send an internal referral to one of the following:
