Cost is just $25 out of pocket, without billing insurance. Walk-ins ONLY. Please bring your school's sports physical forms.

Extended Hours to Better Serve You

Sports Physicals are offered at all Summa Health Urgent Care Centers during the following hours:

• Monday - Friday, 9:00 a.m. - 7:00 p.m.     
• Saturday - Sunday, 9:00 a.m. - 5:00 p.m.
